VIRGINIA SERVICES
Community Guide & Housing Support Services
Available through Northern Virginia CSB partnerships and DBHDS Medicaid Waiver programs
In Virginia, Consulting Connection Services (CCS) provides time-limited Community Guide services in partnership with local Community Services Boards (CSBs), including Northern Virginia CSB.
These services are delivered in accordance with DBHDS Medicaid Waiver requirements and are designed to address immediate housing and community stabilization needs.
Community Housing Guide
Available to:
- Individuals on the DBHDS Medicaid Waiver
- Individuals on the DBHDS Medicaid Waiver Waitlist (via local CSB contract)
Community Housing Guide services are short-term, goal-focused supports that assist individuals in:
- Identifying safe and stable housing options
- Completing housing applications
- Coordinating with landlords and housing authorities
- Understanding lease responsibilities
- Addressing barriers to housing access
- Developing a housing stabilization plan
For individuals on the Waiver Waitlist, services are provided through contract with Northern Virginia CSB to prevent housing instability while awaiting waiver enrollment.
General Community Guide
Available to:
- Individuals currently enrolled in a DBHDS Medicaid Waiver
General Community Guide services are short-term and time-limited supports designed to:
- Connect individuals to community-based resources
- Assist with accessing transportation and local services
- Strengthen independent living skills
- Coordinate with support teams to address identified barriers
These services are structured around specific goals and are not ongoing case management.
DBHDS Housing Referral Waitlist Paperwork Support
CCS also provides assistance with completion and submission of required documentation for the DBHDS Housing Referral Waitlist process.
This includes:
- Reviewing eligibility requirements
- Assisting with required forms
- Coordinating with CSB staff
- Ensuring documentation completeness
